  Certificates in the Allied Health Program (AHE) require seventeen credits of preparatory work before taking any of the classes needed to complete the certificate program. Depending on prior experience, and with AHE faculty advisor approval, some or all of the preparatory work may be waived or substituted.
 INSTRUCTIONS: Take this form to your AHE faculty advisor to indicate the credits to be waived or substituted. Your faculty advisor must sign this form. Submit this completed form to the Financial Aid Services Office.
